Same Day Field Trip with Bus
Have a conversation with Tommy Craig to get initial approval for your field trip.
Complete the Field Trip Request Form
Submit a School Bus Request form to Kimberly Koontz in Attendance Office (minimum of 2 weeks before trip). Click here for a single trip or click here for multiple trips with the same kids.
Complete a permission slip for each attending student.
If your field trip is outside of the school day (8am-3pm), please add this Parent Checklist (Spanish Version).
Contact Nurse Kristen Stoll to get delegated for any medication (Kristen.Stoll@jeffco.k12.co.us) FIRST FIELD TRIP OF THE YEAR ONLY.
Give a copy of the Field Trip Request form and roster(s) of students to Michele Perdue to clear for medical.
Give a copy of the Field Trip Request form and roster(s) of students to Kimberly Koontz and Frostie Peterson to excuse. If students are not going, you must let them know ASAP.
Complete an online High Risk Form as soon as possible ONLY if students are participating in any of the following activities.
Rappelling in Mountains / Rock Climbing
Bungee Runs
Rope Courses
Velcro Walls
Foot Races
Transporting students in private vehicles
Archery
Water sports (Boating and related activity, Water
Skiing/Wakeboarding, Surfing, Scuba Diving,
Swimming and similar)
Inflatable Jumping Structures/ Trampolines
Mechanical Amusement Devices / Rides
Water Parks
Dunk Tanks
Slacklining
Tug of War
Mountain biking
Snow Sports (Skiing, Snow Boarding, Cross
Country Skiing, Snowshoeing, Tobogganing,
Snow Caving, and similar)
Hayrides / Tractor-Trailer Rides
Horseback Riding
Live Animal / Insect Interactions
Bonfires and Other Sources of Fire
District-owned vehicle(s) are being operated out of state.
Scan all completed Field Trip forms and email to Tommy Craig.

**Remember that you must submit a list of attending students to the attendance secretary 3 days before AND on the day of the event.**

Submit a Personal Vehicle Transportation Authorization Form for EACH student traveling in a personal vehicle that is NOT their own. The person they are driving with should be listed at the bottom of this form. (Students driving themselves do not need to complete this form.)
Submit a Confidential Information for Transportation in a Personal Vehicle Form for each driver who will be transporting students other than themselves or parents taking their own student. (All students should complete the student driver section even if they are 18)

Complete the Overnight Travel Planning Checklist including all linked documents.
Submit a School Bus Request form to Kimberly Koontz in Attendance Office (minimum of 2 weeks before trip). Click here for a single trip or click here for multiple trips with the same kids.
International Travel Insurance: All groups must purchase liability coverage which specifically protects the district from claims originating outside the United States. Proof of Insurance coverage, must be provided in advance of travel. Rates will be determined by the District's Rick Management staff but are generally $1.00 per person per day. Request for Travel Insurance Form.

Collect back an Extended Field Trip Medication form for each student, even if they have no meds at least 4 weeks prior to departure. Give these forms to the Nurse Kristen Stoll immediately.
